Course Description and Objectives

This comprehensive transition course is designed to give physical therapists the tools to become active and effective members of educational and health care transition teams that optimize the experience and outcomes for individuals and their families. Using Chapter 34 in the Campbell’s Physical Therapy for Children, 6 edition, along with the ICF model and the Guide to Physical Therapy Practice as frameworks, this course will review and support implementation of the evidence supporting physical therapist participation on transition teams.

The first 90-minute session (Wednesday December 6, 2023 from 7:30-9:00 pm eastern time) will provide key concepts necessary for understanding the issues of youth with disabilities for successful transition along with the introduction of our case for application.

The second 90-minute session (Wednesday January 10, 2024 from 7:30-9:00 pm eastern time) will provide recommendations for expanding the role of physical therapists and optimizing strategies for transition planning in the educational and healthcare system through meaningful evaluation of students/clients.

The final 90-minute session (Wednesday February 7, 2024 from 7:30-9:00 pm eastern time) will provide strategies for optimizing Plans of Care utilizing team processes through the Individualized Transition Plan (ITP) and designing relevant Interventions integrating self-determination for youth success. This interactive presentation will combine lecture and embedded group discussion/brainstorming during each synchronous session.

All participants must complete brief reading and worksheet reflection assignments before synchronous sessions. A brief “capstone” project consisting of the completed worksheet/case reflection is expected at the conclusion of the course to demonstrate application of key concepts.

Learning Objectives

At the completion of this course participants will be able to:

1.  Critically review Chapter 34

2. Describe transition processes in educational and healthcare systems

3. Identify challenges faced by youth and families during transition from pediatric to adult systems

4. Analyze transition approaches, services, supports and outcomes

5. Implement strategies for expanding the role of physical therapists in transition across educational, community and healthcare settings.

6. Identify and access available services/strategies in your local area

7. Apply and Integrate knowledge gained during the course into routine clinical practice.
